UNDER A PHYSICIAN’S ORDER, FOCUS HOME HEALTH, INC. PROVIDES THE FOLLOWING SERVICES:

SKILLED NURSING SERVICES including but not limited to:

• Treatments and injections
• Dressing changes
• Catheter care
• Supervision of medications
• Restorative care
• Respiratory care
• Family/patient health teaching
• Diabetic teaching
• Care of terminally ill
• Drawing blood and lab work
• I.V. fluids and antiobiotic therapy
• Wound care
• Pre-Op and Post-Op care
• Ostomy care

PHYSICAL THERAPY

• Therapeutic exercises related to a specific condition and loss of function
• Utilization of adaptive equipment and aids in rehabilitation, correct and safe home use
• Contribute to the health and well being of patients and their family
• Help restore the patient to health and maximum function
• Prevent costly and inappropriate admissions to hospitals/institutions
• Reduce re-admission to hospitals/institutions
• Make possible earlier discharge from the hospital or nursing home

OCCUPATIONAL THERAPY

• Assist with developing activities and facilitate maximum self care and return to optimal function

SPEECH THERAPY

• Assist patient in regaining speech which may have been impaired following illness or accident
• Evaluate problems related to speech and feeding difficulties

MEDICAL SOCIAL WORKER

• Coordinates services between patient, family, and community resources

NUTRITIONAL GUIDANCE

• Assist patient in understanding, accepting, and following special diets as prescribed by physician

CERTIFIED HOME HEALTH AIDE

• Assist patient with personal care, meal preparation, ambulation, and exercises under professional supervision
